Is the application supposed to write down here in the comment section? or Can we get a copy of ypur application form?

Because we can't quite understand the instructions. Also do you know the one like in the graphic shop? Their application form is like a request where the request was put in another link/website. So I think you should do it too...if you want :) No offense but I hope I can apply asap. :(
